Clinical and Affiliate Faculty Appointment Process

If you would like to be considered for appointment as a Clinical or Affiliate Faculty Member as outlined in our guidelines, please follow the steps below.

  1. Complete this form then submit a recent CV and an optional nomination letter to sopcaf@uw.edu.
    1. Pharmacy residents applying for a clinical faculty appointment need a letter of support and verification from their residency director.
  2. Within a few business days of Step 1’s completion, you will receive an email to complete the sexual misconduct disclosure.
    1. Email will be sent from “UW OAP via Ideal-Logic” to the address provided on the form in Step 1.
  3. Once Step 2 is complete, you will receive an offer letter from sopcaf@uw.edu. Please sign and return to sopcaf@uw.edu.
  4. Application submission is complete.

What Happens Next?

  1. Applications will be reviewed and voted on by the Clinical and Affiliate Faculty committee following the quarterly voting schedule.
  2. If approved, your application will be submitted through the University of Washington’s Human Resources system, Workday.
    1. Processing times vary and we submit the completed applications on a first come, first serve basis after the committee vote. Please allow 3-6 weeks for the appointment to process before you are notified.
  3. If approved by the University, we will send you confirmation and the information necessary for receiving a UW NetID.

Note: Residents with a clinical instructor appointment will not be renewed after they complete their residency program. You are welcome to re-apply, using the same process, for an appointment as clinical assistant professor if you remain involved with UWSOP and meet the appointment qualifications.
