Rank Change Process (All Titles)
CAF Rank Change Request Form (accessible with UW email)
Faculty may remain at each rank indefinitely, contingent upon annual review showing continued service to the Department, but are encouraged to pursue growth and excellence in their field which would enable consideration for rank change.
Clinical and Affiliate Faculty members who are considering a rank change may consult with the CAF Committee and/or the Chair of the Department of Pharmacy (DOP) by emailing sopcaf@uw.edu to discuss eligibility, readiness, and expectations for advancement. To initiate the rank change process, candidates must submit a CAF Rank Change Request Form by September 30, 2026.
Following submission of the request form, eligible candidates will receive instructions for accessing the Interfolio review system and submitting the required dossier materials. Candidates are responsible for preparing and submitting all required documentation, which may include:
- Self-assessment statement describing qualifications for the requested rank. Describe how you meet the criteria for the requested appointment or rank change, specifically addressing your accomplishments and contributions in:
- Teaching, examples may include guest lecturing, student mentorship, experiential education, precepting
- Service, examples may include committee service, community engagement, presentations to student organizations
- Scholarship, examples may include papers, posters, presentations
- Any other contributions as applicable
- Current curriculum vitae (CV).
- A comprehensive and up-to-date curriculum vitae that accurately reflects the candidate’s academic, professional, teaching, service, scholarly, and other relevant activities at the time of submission
- Peer teaching evaluations (if applicable).
- Copies of all available peer teaching evaluations completed for the candidate. Evaluations should be organized in reverse chronological order, with the most recent evaluation presented first.
- Student teaching evaluations (if applicable).
- Copies of all available student, learner, resident, fellow, or trainee evaluations related to the candidate’s teaching activities. Evaluations should be organized in reverse chronological order, with the most recent evaluations presented first.
- Examples of scholarly activity and other supporting materials (if applicable).
- Documentation that demonstrates scholarly, professional, creative, or service contributions relevant to the requested rank. Examples may include publications, presentations, invited lectures, abstracts, book chapters, educational materials, practice innovations, grants, awards, professional leadership activities, or other evidence of impact and achievement.
The School will obtain the required review letters as part of the review process. Candidates are required to identify potential reviewers who can independently evaluate their qualifications for the requested rank. Candidates may provide the names of up to three external reviewers and up to three internal UW School of Pharmacy reviewers, with a minimum of one reviewer in each category required. The School will determine which reviewers to contact and will request the required evaluation letters on the candidate’s behalf.
Once all dossier materials and review letters have been received, applications will be reviewed as follows:
- Applications for rank change to Clinical Assistant Professor or Affiliate Assistant Professor will be reviewed and voted on by the CAF Committee.
- Applications for rank change to Clinical Associate Professor, Affiliate Associate Professor, Clinical Professor, or Affiliate Professor will be reviewed and voted on by the School-based faculty of the Department of Pharmacy.
Following the review and vote, the Chair of the Department of Pharmacy will review the application and provide a recommendation to the Dean. The Dean will review the recommendation and supporting materials and will make the final decision on the proposed rank change in accordance with University and School policies.
