1. Registration

Deadline: 8/18/23
Registration: https://forms.gle/QuxWdUvLJ6xadXcz6
Please note that some institutions (Providence) might block access to Google Docs, so you may need to access this link off site or through a personal device/data connection.

Other requirements:
Residents must receive a UW Net ID in order to access course information via CANVAS.  This is obtained as part of obtaining a Clinical Appointment with the UW SOP.  Click here for details.  Due to UW HR databases being changed this year, forms need to be sent in time for the last vote on May 26th, otherwise applicants may not be processed until after July 7th, preventing CAF appointments through August.  (Meaning forms need to be in earlier than May 25th to meet the May 26th deadline).

2. Payment

Deadline: AUG 10, 2023 (payment after this date will incur late fees)
Payment site: https://washington.irisregistration.com/Form/5589
Fees for 2023 will be paid online via Credit Card or by UW budget code.   Late registration fees will be charged after mid August, 2023 (as outlined on the payment site).  The payment system will ask you to create an account to track your payment/receipt.  This does not need to be tied to a UW email address.

For UW budget codes – please have your financial representative from your department contact Alex Tu khoifish@uw.edu.
