Have you changed your name? We will want to get your correct name on the University database as soon as possible. To make this change, we will need two things from you: a photocopy of your social security card with the new name on it, and a filled out personal data form. All of the courses you will be teaching have preceptor's guides available and to download these, you will need to check our courses and syllabi page.
