Clinical and Affiliate Faculty Information

If you are a clinical or affiliate faculty member of the Department of Pharmacy at the University of Washington School of Pharmacy, or a preceptor for students on rotation from the University of Washington School of Pharmacy, welcome! This page is meant to provide you with information that we hope you will find useful. If you think of information that is useful but not here, please email us (email link at the bottom of this page).

If you would like to become or to nominate someone as a clinical or affiliate faculty member, or would like to be promoted, you will want to read the clinical and affiliate faculty appointment and promotion criteria to see whether you meet the criteria. Visit this webpage for more details.

Have you changed your name? We will want to get your correct name on the University database as soon as possible. To make this change, we will need two things from you: a photocopy of your social security card with the new name on it, and a filled out personal data form. Fax both of these to our office at 206-685-9297 or scan and email the resulting files to us and we'll try to make those changes as soon as possible. Please include your old name on the cover sheet so that we don't get confused!

All of the courses you will be teaching have preceptor's guides available and to download these, you will need to check our courses and syllabi page. For information about topics taught in the third-year therapeutics series, you can consult the lecture notes at the Therapeutics home pages for PHARM 560, PHARM 561, or PHARM 562. Some of the other courses also have notes and information online, and you can link to this information from the student courses home page.

If you would like information on positions available in pharmacies in the Puget Sound area, we do not have a job line available, but you can check the home pages for the Washington State Pharmacy Association to view their listings. We do have information for you if you are thinking about updating your résumé or want to assemble a cover letter for a job application. This information is geared toward students, but pharmacists will find it useful too.
